Symposium Fees and Registration*

Early Registration, including applicable payment, must have been received by the Symposium Office no later than May 20, 2009 in order to qualify for early registration fees. All others must nowregister ONSITE and are subject to onsite fees below. We are no longer accepting early registrations. We are sorry, but early regisration requests received after this time CANNOT BE PROCESSED.

DO NOT mail, FAX or email this form to the Cord Blood Symposium, as it will not be processed. It must be presented ONSITE. Please be aware that onsite registration fees are NON-REFUNDABLE.

* All registrations include breakfast, lunch and coffee breaks for each day registered. All registrants will receive a CD containing slides and audio of consenting speakers' presentations, mailed approximately 6-8 weeks following the symposium.

†CBBS Individual Membership must be current for 2009 to qualify for member pricing. Your Individual Member ID Number will be required to register as a CBBS Member.

• Registrants may receive a refund of their registration fee minus a $40 administrative fee in the event a written notice is received of cancellation no later than May 13, 2009. We are sorry, but no refunds will be issued after this date.
